8 for 8 Program Enquiry Form

    This form is for enquiries for the 8 for 8 program only. To register for the 8 for 8 program, the parent, guardian or participant must visit the bowling centre to finalize the registration process. This visit will allow us to introduce the participant to our centre and complete the official 8 for 8 program registration form.
